Office Policies

Thank you for choosing Rawle Orthodontics for your orthodontic needs. Please be aware of the following office policies regarding your (or your child’s) orthodontic treatment.

Financial Policies

  • When reviewing the financial agreement, please take note that your monthly payment will be automatically deducted from your debit card or processed to your credit card. A $30 charge will be made to the responsible party’s account for any notification of insufficient funds debit or credit card transaction which has been declined. No checks will be accepted.

  • Payments are processed normally on the 1st or the 15th of the month. If funds are not available, a late charge of $15 will be assessed. If payment is 30 days delinquent, a 30 day letter will be sent.

  • If the account falls 60 days delinquent, late charges are applied on both late payments. No active treatment will occur until the account is brought up to date. The responsible party must speak to our financial coordinator to make arrangements for bringing the account current.

  • If the account falls 90 days delinquent, further late charges are applied and a DISMISSAL letter will be sent by certified mail. This will terminate our responsibility to the patient and will require treatment to be rendered by another orthodontist. However, if the patient or responsible party pays the entire unpaid balance, active treatment can be reinstated.

  • A monthly payment is simply a convenient way to spread out the cost of orthodontic treatment and is not related to treatment progress or appointments; therefore, payment is still due even if there is no appointment during the month. The balance of the total treatment fee is due before the appliances are removed.

  • If you transfer during the course of treatment, your account will be prorated, and the account must be settled prior to sending your records to your new orthodontist.

Insurance Policies

  • Your insurance company will give us an estimate of the amount they will be paying but it is NOT a guarantee. The amount paid will depend on whether the patient is still eligible for coverage while active treatment is still being rendered, and, whether the lifetime orthodontic benefit has been exceeded.

  • If for any reason during active treatment the insurance benefits decrease, change or are terminated, the unpaid balance will be transferred to the patient’s ledger and will be the responsibility of the patient. Any unpaid insurance balance at the end of the treatment is the patient’s responsibility and must be paid before appliances are scheduled to be removed.

  • The responsible party must inform our office of any change in coverage or insurance company within 30 days of the change, failing which the balance will be transferred to the patient’s ledger and will become the patient’s responsibility.

  • If the account falls 90 days delinquent, further late charges are applied and a DISMISSAL letter will be sent by certified mail. This will terminate our responsibility to the patient and will require treatment to be rendered by another orthodontist. However, if the patient or responsible party pays the entire unpaid balance, active treatment can be reinstated.

  • We reserve the right to refuse to process/accept insurance benefits at any time.

Rawle Orthodontics is committed to helping you maximize your insurance benefits. Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. Your estimated patient portion must be paid at the time of service. As a service to our patients, we will bill insurance companies for services. If you have any questions, our courteous staff is always available to answer them.

Additional Charge Policies

  • Broken appointments without a 24-hour advance cancellation notice will be assessed a fee of $25 for short appointments (20 minutes or less) and $50 for long appointments (more than 20 minutes).

  • Broken appliances which are the result of poor patient cooperation will be assessed a repair fee as follows:

    • $25 per broken bracket, $25 per broken wire.

    • $50 for Herbst, Expander, Mara, Forsus, Headgear, Tongue crib, Bite plate, Pendex, Space Maintainer/Space Regainer.

  • If the appliance cannot be repaired and needs to be replaced, the full replacement fee will be charged for a new appliance based on the cost at the time of replacement.

  • The fee for the repair or replacement of an appliance is due at the time of repair or replacement.

  • Treatment which has extended two months past the initial estimated date of completion due to non-compliance by the patient will result in a fee of $150 per visit for each additional visit needed for treatment to be completed. Our services may be discontinued for lack of cooperation solely within the office’s discretion.

  • The fee for treatment includes one set of Essix retainers. If you require another set because they are lost, damaged or for any other reason, there will be a charge for new retainers. If you request bonded retainers, there will be an additional cost for each bonded retainer.

Treatment Cooperation Policies

Successful and timely treatment is dependent on the following:

  • Broken appointments without a 24-hour advance cancellation notice will be assessed a fee of $25 for short appointments (20 minutes or less) and $50 for long appointments (more than 20 minutes).

  • Patient cooperation in elastic wear

  • Proper oral hygiene

  • Eliminating foods and eating habits that can break or distort appliances

  • Reporting broken or lost appliances promptly and in advance of scheduled appointments

Appointment Policies

Appointments are normally scheduled in six (6) to eight (8) week intervals. In order to provide quality orthodontic care, it is necessary to schedule some appointments during school/work hours. Appointments an hour or longer will be scheduled in the morning. We understand that this can be inconvenient for some patients. However, it may be necessary to schedule lengthier appointments during school/work hours to achieve the best possible treatment.

It is up to the patient to schedule his/her own appointments on a timely basis. These can be scheduled when leaving after each visit and are therefore scheduled in advance. A twenty-four (24) hour notice is required to cancel an appointment.If an appointment is missed, it is the patient’s responsibility to contact our office immediately to set up a new appointment.

As a courtesy to you, an automated system will confirm your appointment via text message, 1-2 days prior to your appointment. Please do not rely on this as the only means of remembering your appointment as there could be technical circumstances that may prevent a confirmation from going out.

Dental Care Policies

The orthodontic fee does not include dental check-ups, cleanings, fluorides or other preventive, restorative, cosmetic procedures or extractions. The fee for any additional services provided by our office, your family dentist or a dental specialist will be determined by those providing the care. We advise patients to see their general dentist every three (3) months during active orthodontic treatment. In the event that you have changed dentists or there has been a change in medical history, please notify our office as soon as possible.

After your initial examination with Dr. Rawle, one of our friendly staff members will speak with you to help you determine which of the following financing options you prefer:
